Tell us what is happening

The first conversation is an opportunity to describe the person who needs support, what has changed, what the family is currently managing, and what kind of help you believe may be needed. You do not need to have every answer before calling.

Clarify the needs and constraints

A Client Care Coordinator will complete or coordinate the home-care assessment, involving Northline’s dedicated nurse when clinical knowledge or nursing judgment is appropriate. The assessment covers daily routines, personal-care needs, mobility, memory, household expectations, location, desired schedule, timing, and other factors that affect service.

Determine whether Northline is the right fit

Sometimes the request fits traditional home care. Sometimes the person needs skilled nursing, healthcare advocacy, a facility, or another type of support. Honest guidance at the beginning is better than beginning an arrangement that cannot meet the actual need.

Develop a practical service plan

If Northline is appropriate, we establish the requested responsibilities, schedule, communication preferences, and service expectations. Northline is designed primarily for clients who need at least 40 hours of care each week, generally in 8- to 12-hour shifts. Smaller schedules may be considered in limited circumstances. We will discuss the schedule and other factors that may affect timing and Care Professional availability.

Prepare to begin care

Northline identifies Care Professionals who can meet the practical requirements of the assignment and prepares them with the information needed to begin. Starting service may happen quickly in some circumstances; other situations require additional time to build responsibly.

Communicate and adjust

As needs or circumstances change, your Client Care Coordinator works with Northline’s Administrative and Scheduling team to understand what is different, keep you informed, and determine what adjustments may be available. Other team members may assist when appropriate.
